The patient has an one-piece intraocular lens, presents with an intraocular pressure of 30, and the following is seen on slit lamp examination. What's the diagnosis?

We congratulate Dr. Elisa Skenduli-Bala! The answer is pigment dispersion syndrome, a.k.a. "peppered iris syndrome" from implantation of a single-pieced IOL in the ciliary sulcus. Single-pieced IOLs, such as the Alcon AcrySof SA60AT, should only be placed in the lens bag.
